There are three items that are required to enroll a student in the Portales Municipal School District. These requirements are based on laws that must be followed to ensure the safety of your child.

  1. Original Birth Certificate

    • Power of Attorney is required if the student is being enrolled by someone other than the person listed on the birth certificate.

    • Power of Attorney (if needed)

    • You will need Adobe Reader in order to view the PDF files. If you do not have Adobe Reader you can download that software Here

 

  1. Current Shot Record

    • Copy from previous school is acceptable


  1. Primary Phone Number

    • School Messenger is used in our district to contact parents/guardians in case of emergency and for many school announcements such as cancellations and delays. We must have a means to contact parents/guardians in an emergency.
